2025 Open Enrollment

American Fidelity's open enrollment will be held from October 20, 2025 to November 14, 2025.  Plan changes will take effect January 1, 2026.

What do you need to do?

All full-time active 欧美群交 employees, working 30 hour per week or more, must enroll in benefits for the new calendar year with American Fidelity. Your current enrollments (including dependents) will not roll over to the next year. To help with the transition, please schedule an appointment with an American Fidelity representative that will help you understand the plans and make a decision.

New Base Plan options for you and your family

Medical through SISC
  • Blue Shield PPO 80K
  • Blue Shield PPO 80M (HRA included)
  • Blue Shield HSA $5,000 (HSA included)
  • Kaiser HMO $10 OV (HRA included)
Dental through Delta Dental (ACSIG)
  • Same benefits as your current plan
Vision through VSP (ACSIG)
  • Same benefits as your current plan

Basic Life & Long-term Disability through Lincoln Financial Group

  • Review and elect your new benefit options via our online platform
  • Ask questions about selecting a new health plan
  • Provide supporting documentation for all dependents

These information sessions are a great opportunity for you to visit with insurance carriers and ask any specific questions you may have regarding your health benefits.

NOTE: These info sessions DO NOT replace the 1:1 session. You still need to meet with an American Fidelity Representative.

Spouse
  • Prior year鈥檚 Federal Tax Form that shows the couple was married (First page only, financial information may be blocked out).
  • A marriage certificate will be accepted ONLY for newly married couples when filing has not yet been required for the current tax year.

Domestic Partner

  • A Certificate of Registered Domestic Partnership issued by the State of California or a certified copy of the Declaration of Domestic Partnership that includes the dated, signed Secretary of State Certification Stamp. (Enrolling a Domestic Partner may cause the employer contribution to become taxable.)
Children, Stepchildren, and/or Adopted Children up to age 26
  • Legal Birth Certificate or Hospital Birth Certificate (to include full name of child, parent(s) name & child鈥檚 DOB)
  • Legal Adoption Documentation
Legal Guardianship up to age 18
  • Legal U.S. Court Documentation establishing Guardianship
Unmarried Disabled Dependents over age 26 (required enrollment in a SISC medical plan)

BLUE SHIELD (All items listed below are required)

  • Legal Birth Certificate or Hospital Birth Certificate (to include full name of child, parent(s) name and child鈥檚 DOB)
  • Prior year鈥檚 Federal Tax Form that shows child is claimed as an IRS dependent (First page only, income information may be blocked out)
  • Proof of 6 months prior creditable coverage under the employee/retiree鈥檚 plan. There can be no break in coverage.
  • Completed Declaration of Disability for Overage Dependent Child

KAISER (All items listed below are required)

  • Legal Birth Certificate or Hospital Birth Certificate (to include full name of child, parent(s) name & child鈥檚 DOB)
  • Prior year鈥檚 Federal Tax Form that shows child is claimed as an IRS dependent (First page only, income information may be blocked out)
  • Proof of 6 months prior creditable coverage under the employee/retiree鈥檚 plan. There can be no break in coverage.
Retirees and/or Dependents on a Retiree Plan Age 65 or Over
  • Proof of enrollment in Medicare Part A & Part B (copy of current Medicare card or Medicare enrollment confirmation letter showing effective dates of Part A and Part B)

Q:  Do I need to meet with American Fidelity during the enrollment? 
A:   Yes. Because this is a new system and data accuracy is vital, we are requesting that all employees meet with an American Fidelity representative this year to complete their health and/or supplemental benefits enrollment.

Q:  When will appointments be available with American Fidelity?鈥 
A:   The enrollment will begin October 20th, 2025, and continue through November 14th, 2025. American Fidelity representatives will be on site to meet with employees. An up-to-date schedule and option to schedule your appointment is available in the "In-Person Schedule" tab.

Q:  What will be covered during the meeting with American Fidelity? 
A:   An American Fidelity representative will confirm your medical, dental, and vision benefits for the upcoming plan year. They will also review pre-tax options, including Flexible Spending Accounts, Health Savings and HRA accounts. You must sign a new pre-tax election form with American Fidelity to continue receiving pre-tax benefits. As this is the first year on our new secure online system, during your appointment they will take your census information.

Q:   What do I need to bring to my appointment with American Fidelity? 
A:    If you are adding a spouse or dependent child, additional documents may be required. You will need dates of birth and social security numbers for dependents who are being covered on your benefits. See "Required Documentation" tab.

Q:   Will I have the option to find out more about additional supplemental benefits? 
A:   Yes, you鈥檒l be able to learn more about benefit options that could be important to you and鈥痽our family, such as disability insurance, life insurance, annuities, and accident protection.鈥

There is no obligation to purchase supplemental insurance from American Fidelity. 

Q:   What is an HRA?
A:    A Health Reimbursement Account (HRA) is an employer-funded account that reimburses employees for qualified medical expenses. Unlike an HSA or FSA, employees do not contribute to an HRA - the funds are provided solely by 欧美群交. For more information, visit

Q:   How does an HRA work?
       1. 欧美群交 sets aside a specific allowance for the plan year. In this case, $200 per month.
       2. You pay for eligible medical expenses out of pocket.
       3. You submit receipts or claims for reimbursement.
       4. Once approved, you are reimbursed tax-free, up to the available balance in the account.

Q:   What types of expenses are covered?
A:    HRAs typically cover IRS-approved medical expenses, which may include:

Deductibles, copays, and coinsurance
       鈼
Prescription drugs
       鈼
Medical, dental, and vision expenses
       鈼
Certain over-the-counter items

Q:   Do I need to be enrolled in a specific health plan to use the HRA?
A:    Yes. You are eligible to receive the HRA if you enroll in one of the following:

Blue Shield PPO 80M
       鈼
Kaiser HMO $10

欧美群交 contributes $200 per month into your HRA.

Q:   Do unused HRA funds roll over to the next year?
A:    Yes, as long as you remain enrolled in a qualified plan such as the Blue Shield PPO 80M or the Kaiser HMO $10.

Q:   How do I get reimbursed?
       鈼 Submit a claim through your employer鈥檚 administrator (online portal, app, or paper form).

Provide receipts or Explanation of Benefits (EOBs) as proof.
       鈼
Reimbursements are usually made via direct deposit or check.

Q:   How is an HRA different from an HSA or FSA?
       鈼 HRA: Funded only by the employer; no employee contributions
       鈼
HSA: Employee and/or employer can contribute; portable; must be paired with a high-deductible health plan
       鈼
FSA: Funded by employee pre-tax dollars (sometimes with employer contributions); 鈥渦se-it-or-lose-it鈥 rules usually apply

Q:   What happens if I change from and HRA eligible health plan to a non-HRA eligible health plan?
A:    If you switch to a non-HRA eligible health plan, any remaining funds in your HRA will not transfer your new plan. HRA funds are only available while you are enrolled in an HRA-eligible health plan and cannot be rolled over or used once you move to a non-HRA eligible health plan option. Be sure to use any remaining HRA funds before your HRA coverage ends.

Q:   What can I do with an HSA?
A:    A Health Savings Account (HSA) allows you to use funds you set aside plus Hartnell鈥檚 monthly contribution to pay for qualified medical, pharmacy, dental, and vision expenses鈥攐r save and grow those funds for future use. 欧美群交 contributes $350 per month into your HSA.

Q:   How do I use an HSA?
       鈼 You can pay directly with your HSA debit card at the time of service or purchase

Or, pay out-of-pocket and reimburse yourself later

Make sure to keep all receipts and Explanation of Benefits (EOB)

Q:   How can I open an HSA?
A:    You can open an HSA with American Fidelity. For more information, you can visit

Q:   What is a qualified expense?
A:    Qualified expenses are IRS-approved medical, dental, vision, and prescription costs. Many over-the-counter (OTC) items also qualify.

Eligible expenses include:

Acupuncture

Birth control treatment

Blood sugar test kits for diabetics

Breast pumps and lactation supplies

Crutches

Dental treatments (including X-rays, cleanings, fillings, sealants, braces, and tooth removals)
